To place an Order use this Secured Order Form or place a phone order. We accept phone orders Monday through Saturday 10:00 am to 6:00 pm Sunday 10:00 am to 3:00 pm Eastern Standard Time 330.427.6211. We are located in Ohio.
Payment by Visa, MasterCard, Discover or American Express.
We ship within the Continental U.S.A. ( 48 states). We will ship to our Military APO/FPO with Priority Mail.
Order items by the Item Numbers. We do Not replace missing or stolen orders.
This order form is for Wholesale Samplers or Wholesale Cases. Minimum order for cases is one case.
Ohio Customers Only: If you are Sales Tax Exempt we require you to provide us with your Ohio Vendor's License number on the order form below. All other Ohio customers will be charged 7.25% Ohio Sales Tax.

Shipping Samples

Sampler Assortments are shipped Priority Mail or First Class Mail within 24 hours of order Monday through Saturday. Shipping fee is included in the cost of the Samplers.

Sales Receipts

Wholesale customers, your Sales Receipt will be Emailed to you. Hard copies of your Sales Receipts are packed in the shipping carton. Make sure you retrieve your receipts for your records.

My Ups Number Ups Zip Code		Wholesale customers. If you or your company have a valid Ups number we will gladly ship your order using your Ups account number. In order to ship using your Ups number you must enter the zip code to where you actually receive your Ups bill and your valid Ups number.
Ohio Vendors		Ohio Customers Only. Ohio customers wanting to purchase Wholesale Cases must provide us with their Ohio Vendor's License Number for us to process the order. If no number is entered in this field you will be charged 7.25% Ohio Sales Tax.

Shipping Fees
Wholesale Cases

Wholesale cases are shipped Ups ground Monday through Friday. We must pull and weigh your order to get the amount of the shipping charge. Whenever possible we bundle cases to get the lowest possible shipping rate for our customers. You will receive an email Order Confirmation from us with your order number, ship date, shipping fee and total. Once your order has shipped you will then receive an email from Ups with your tracking number (s). Ups will not deliver to PO boxes. You may also request a Shipping Quote prior to placing an order, email or call us.
